Job summary

Devotion Hospice in Westminster, CO is seeking a part-time Occupational Therapist to support our home health patients. You will assess, plan, and implement therapy to improve independence, safety and functional mobility across age groups in a home care setting.

Requirements include a valid OT license and CPR certification, plus a valid driver’s license with auto insurance. HomeCare HomeBase experience is preferred and one year of OT experience is advantageous.

Qualifications

• Licensed OT required.
• CPR certification required.
• Valid Driver’s License and Auto Insurance required.

Responsibilities

• Perform clinical assessments and create plans of care.
• Document progress notes and plans of care in a timely manner.
• Accept assignments within education and competence.
• Participate in IDG care planning process.
• Apply infection control and standard precautions during patient care.
• Assess ROM, strength, coordination, ADLs and safety, and adapt equipment as needed.
• Facilitate communication within the care team to achieve outcomes.
• Evaluate occupational performance in self-care, work, leisure and rest.
