Qualifications

• Bachelor's degree in occupational therapy from an accredited college or university is required.
• Academic or professional doctorate (PhD, DSc, OTD) preferred.
• Current Arizona licensure as an Occupational Therapist.
• Basic Life Support certification required at hire and valid for 90 days or greater.
• Maintains Basic Life Support (BLS) competency.

Responsibilities

• Performs evaluation/re-evaluation and develops plan of care.
• Provides evidence-based treatment and assesses data reflecting patient status.
• Maintains accurate documentation and appropriate billing.
• Engages in research, education and quality initiatives; pursues professional growth.
