Job summary

Premise Health in Indiana is seeking a Part-Time Nurse Practitioner to provide care to employees and dependents in our Health Center in the greater Elkhart area. Schedule: Tuesdays, 8:30am-4:00pm. Primary care experience, chronic condition management, and DEA required.

Responsibilities include exams, procedures, care plans, and collaboration with clinicians to ensure comprehensive, high-quality care.

Qualifications

• State RN and NP licenses required.
• DEA license required; prescriptive authority where applicable.
• Master’s Degree in Nursing with current credentials.
• National certification (ANCC or AANP) required.

Responsibilities

• Conducts physical exams and preventive health measures per guidelines.
• Performs procedures such as injections, suturing, wound care, and infection management.
• Develops patient care plans, records progress, and ensures continuity of care.
• Educates patients on treatment plans, health maintenance, and development.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Prescribes medications and therapies as appropriate.
• Refers patients to specialists as needed.
• Maintains patient health records with accurate documentation.
• Educates and coaches nursing staff on best practices.
• Performs other duties as assigned.
