Optum is seeking a Part time – Nurse Practitioner – Traveler to join our HouseCalls team in West Virginia. Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks, and coordinate follow-up care to improve health and well-being.

Our integrated medical teams in Home and Community Care are innovating healthcare by providing high-end medical services, compassionate care, and industry-leading solutions to vulnerable populations. Our holistic approach addresses physical, mental, and social needs, helping patients access and navigate care anytime, anywhere.

This role offers work-life balance with flexible scheduling over a 7-day week, no on-call, weekends, or holidays required.

The position requires travel across West Virginia, approximately 85% of the time. All travel expenses are covered, with a base salary plus incentives.

Primary Responsibilities
  1. Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  2. Perform screenings and point of care testing as appropriate.
  3. Identify diagnoses for care management and treatment planning.
  4. Summarize medical conditions based on assessments.
  5. Communicate findings to inform the patient’s PCP.
  6. Identify urgent situations and intervene appropriately.
  7. Educate members on disease management, medications, and compliance.
  8. Travel at least one week (40 HPW) at a time.

Required Qualifications
  1. Active NP license or ability to obtain by start date, with no disciplinary restrictions.
  2. Autonomous practice approval if applicable, with application deadlines specified.
  3. Active national certification in relevant specialties or ability to obtain it.
  4. Physical ability to perform assessments and handle physical tasks.
  5. Reliable transportation for travel within the state.
Preferred Qualifications
  1. Over 1 year of clinical experience.
  2. Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  3. Home health or home visit experience.
  4. Additional licensure in other areas may be required.
  5. Employees must commit to at least 12 months in the role; full loan repayment benefits require 36 months of service.
