Responsibilities

• Serves in the role of primary care provider to long and short term residents; assessing health by visiting patients and performing physical examinations
• Team based care, collaborating with the medical attending/physician and nursing home staff
• Educating residents and families, formulating evidence-based care plans
• Complies with federal, state, and local requirements; advising management on needed actions
• Maintains professional knowledge through education and professional activities
• Ensures safe and clean working environment aligned with organizational values
• Reviewing and ensuring charting, documentation, and care administration are up to date
