Job summary

AbsoluteCARE Medical Center & Pharmacy in Philadelphia seeks a Complex Care Nurse Practitioner to join our outpatient care team. You will provide assessments, counseling, and care planning for patients with complex chronic conditions in a collaborative setting with physicians and multidisciplinary staff.

The role emphasizes transition care, hospital discharge coordination, and management of acute exacerbations in outpatient settings with IV hydration, antibiotics, and wound care under physician

Qualifications

• Licensed Advanced Practice Provider in the state of practice.
• 2 years of experience as an Advanced Practice Provider in Primary Care.
• DEA license.
• ACLS certification.
• Experience with patients who have multiple chronic medical conditions.
• Experience with Medicare, Medicaid, and commercial populations.
• Knowledge of chronic disease management programs and services.
• Proficiency with EMR; eClinicalWorks preferred.

Responsibilities

• Manage and improve health for patients with multiple chronic medical conditions.
• Manage a panel of approximately 200 to 250 chronic care ambulatory patients in an office-based setting.
• Work in a team environment with social work, case management, behavioral health, pharmacy, and nutrition; collaborate with supervising physician.
• Develop transition care plans and track paned patients along the continuum of care including discharges from hospital, ER, SNFs, and home care.
• Communicate with other care facilities to facilitate appropriate discharge or return to AbsoluteCare Medical Center.
