Optum Nurse Practitioner - Home-based Medical Care, Erie, PA

Optum is seeking a Nurse Practitioner to join our Home-based Medical Care team in Erie, PA. We are a clinician-led care organization dedicated to creating a seamless health journey for patients across the care continuum.

As part of the Home and Community Care team, you'll help deliver home-based medical care to complex, chronic patients, helping older adults spend more days at home.

Position Overview

This role involves working as a provider within the HBMC interdisciplinary team to deliver home-based medical care within an assigned geography. The position collaborates with PCPs, interdisciplinary teammates, consultants, and other healthcare professionals to develop and implement patient care goals throughout their journey.

Key Responsibilities
  1. Perform initial assessments and ongoing care for enrolled patients.
  2. Develop and update patient management care plans.
  3. Review care plans with patients and/or legal representatives, involving PCPs when applicable.
  4. Perform basic nursing procedures (IV, blood draws, injections, wound care, etc.) with training provided.
  5. Prescribe diagnostics and interventions to prevent unnecessary hospitalizations.
  6. Conduct follow-up and post-discharge assessments.
  7. Coordinate with hospital, emergency, or post-acute teams following patient transfer.
  8. Educate patients and families on disease management and wellness.
  9. Participate in patient care and clinical excellence meetings.
  10. Implement quality initiatives like HEDIS measures.
  11. Participate in on-call rotations during and after business hours.
  12. Stay current with medical research, technology, and best practices.
  13. Practice in accordance with state laws and regulations for advanced practice nursing or physician assistants.
Qualifications
Required
  • Completion of an accredited Nurse Practitioner or Physician Assistant program.
  • Current RN/NP or PA license in the state.
  • Certification from AANP, ANCC, or NCCPA.
  • Reliable transportation and compliance with vehicle safety policies.
Preferred
  • 2+ years of clinical experience in primary care, home-based care, geriatrics, or urgent care.
  • Knowledge of long-term care, community resources, and cost-effective care delivery.
  • Familiarity with CPT, ICD-10, HCPCS codes.
  • Understanding of clinical standards, NCQA, CMS, Medicaid/Medicare.
Compensation and Benefits

The annual salary ranges from $104,500 to $156,000, based on full-time employment, experience, and performance. Benefits include a comprehensive package, incentive programs, stock purchase options, and 401k contributions. Benefits are subject to eligibility.
