Responsibilities

• Promote independence: Assist with activities of daily living, community living skills and rehabilitation goals.
• Health & Wellness: Assist with medication administration, monitor nutrition plans, and prepare well-balanced meals daily.
• Proactive Behavioral Support: Support clients through challenging situations and implement behavior plans.
• Observation & Advocacy: Identify changes in clients' conditions and alert the treatment team.
• Transportation & Community Outings: Safely transport clients to medical appointments and outings.
