Theoria Medical in Mishawaka, IN is seeking a part-time Medical Director and Attending Physician to oversee care at local skilled nursing facilities. You will lead clinical teams, provide high-quality primary care, and ensure seamless coordination across resident care.

Qualifications

• Active U.S. medical license and board certification required.
• New graduates who are board-eligible encouraged to apply.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, PM&R, and Geriatric Medicine.

Responsibilities

• Serve as the attending physician for patients at assigned SNFs.
• Lead clinical supervision and collaborate with NPs/PAs for daily patient care.
• Provide high-quality, patient-centered primary care for acute and chronic conditions.
• Participate in chronic care management, transitional care, and remote monitoring programs.
