Job summary

Sevita, a leading provider of home and community-based health care, is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a part-time role in Texas. Provide nursing services, training, and supports to address medical or psychiatric needs of individuals with brain injuries and developmental disabilities.

You will monitor health, administer meds, conduct assessments, and coordinate with physicians and agencies to ensure consistent, quality care.

Qualifications

• Associate’s Degree in Nursing or state nursing certificate.
• One year of nursing experience with the target population.
• Current LPN or LVN licensure.
• Valid driver’s license, car registration, and auto insurance.
• CPR/FA and HIV/AIDS, BBP, ACT trainings as required by state licensing.
• Ability to effectively communicate with staff, individuals served, and families.

Responsibilities

• Monitor health conditions and ensure quality services.
• Administer medications and supervise staff administering meds.
• Provide prescribed medical treatment by physician orders and personal care services.
• Conduct assessments and respond to changes in health conditions.
• Provide staff training on infection control and medication administration.
• Coordinate with medical providers and external agencies to ensure consistent care.
• Educate individuals served on self-medication, treatments, sex education/STD prevention, and smoking cessation.
