Job summary

HealthPro Heritage in Commack, NY is seeking a Part-time or PRN Licensed Physical Therapist Assistant (LPTA) or Physical Therapist (PT) for an assisted living community. Therapy services are provided Monday through Friday; no evenings or weekends.

• Associate degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Excellent communication and interpersonal skills; able to work collaboratively

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists.
• Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Document patient treatments, progress, and responses accurately in facility records.
• Collaborate closely with Physical Therapists and the care team to ensure coordinated care.
