Job summary

Cheshire House Nursing and Rehabilitation Center in Waterbury, CT is seeking a compassionate Licensed Practical Nurse (LPN) for a part-time role, 16 hours weekly, to support residents with high-quality, person-centered care in a collaborative team.

You will provide direct nursing care under RN supervision, administer medications and treatments, monitor vitals, assist with ADLs, and ensure accurate documentation of observations.

Qualifications

• Valid, unrestricted LPN license in Connecticut.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS certification as required by facility policy.

Responsibilities

• Provide direct nursing care to residents under supervision of an RN or physician orders.
• Administer medications and treatments per orders and facility policy.
• Take and record residents' vital signs and monitor for changes in condition.
• Assist with activities of daily living (ADLs) as needed.
• Perform basic wound care and dressing changes as allowed by policy.
• Collect specimens for lab tests as ordered.
• Observe and document resident status and report changes to RN/physician.
• Maintain accurate medical records and communications with care team.
