Job summary

Grace Community Care & Homes is seeking a Licensed Practical Nurse for private duty nursing in New Jersey. The role supports person-centered services, reliable operations and accurate documentation.

You will provide nursing care in client homes, follow care plans, document services and collaborate with intake, credentialing, billing and quality teams. This is a part-time position with schedule discussed during hiring and start date contingent on program approvals.

Qualifications

• Active LPN license in New Jersey.
• Experience in home health or private duty nursing preferred.
• Ability to follow care plans and document services accurately.

Responsibilities

• Provide assigned nursing care in client homes under required direction; follow care plans, document services and elevate clinical changes.
• Collaborate with department leadership, intake, credentialing, billing and quality teams; maintain confidentiality and communicate service gaps.
• Participate in orientation, competency review and quality improvement.
