Job summary

AvoCare is seeking a compassionate Registered Nurse (RN) for part-time home-based palliative care in Glendale, AZ. Qualifications

• Arizona RN license or valid multi-state license with Arizona eligibility.
• Minimum 3 years of full-time equivalent nursing experience.
• Active CPR/BLS certification may be required.
• Clinical competency and ability to work in home-based palliative care.

Responsibilities

• Provide home visits for palliative patients and symptom management.
• Document assessments and plans in EMR after each visit.
• Coordinate with healthcare team and insurance case managers.
• Attend regular staff meetings and participate in quality improvement.
