Job summary

Rochester Regional Health is seeking an Licensed Practical Nurse for the Home Care Team as a part-time Case Assistant in Rochester, NY. The role coordinates patient services, schedules, and referrals, with 20 hours per week and day/evening shifts (Fri 12pm-5pm, Sat 8am-5pm, Sun 8am-4pm).

A signing bonus up to $15,000 is offered. Requirements include graduation from an accredited LPN program and NYS LPN license.

Qualifications

• Graduate of an accredited school of licensed practical nursing.
• A valid NYS license as a Licensed Practical Nurse.
• One year of home care or medical office experience preferred.
• Excellent interpersonal and customer service skills.
• Ability to multi-task in a fast-paced environment.

Responsibilities

• Validates supplemental orders and processes per policy.
• Remains available to assist in other duties defined by the LPN scope.
• Manages multiple phone calls with timely follow-up.
• Provides field staff assistance and resources as directed.
• Updates the EMR per agency standards.
• Transfers or discharges as required by regulation.
• Builds strong, communicative relationships with the team.
• Provides exemplary customer service.
• Performs general office duties and documentation tasks.
