Job summary

Graceworks Lutheran Services is seeking an LPN for Bethany Village in Centerville, OH. The role involves assessing and coordinating nursing care for a resident caseload, administering medications, and documenting per Medicare/Medicaid and facility guidelines.

The full-time position offers evenings (3:00 pm – 11:30 pm) with a $10,000 retention bonus for 2nd and 3rd shifts, plus shift premiums and benefits. Previous long-term care experience is preferred but not required.

Qualifications

• LPN license from an accredited school of nursing.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for a caseload of residents from admission to dismissal on assigned nursing unit.
• Coordinate and supervise nursing care delivered by nursing assistants on the unit.
• Administer and chart medications and treatments per procedures.
• Make rounds on each resident and assess for significant changes in condition.
• Supervise nursing assistants; document and report to the interdisciplinary team.
• Complete nursing notes according to Medicare/Medicaid and Bethany Village guidelines.

10K Retention Bonus for Full time 2nd and 3rd shift positions

Responsible for assessing, planning, implementing, and evaluating the nursing care for a caseload of residents from admission to dismissal on assigned nursing unit. Coordinates and supervises the nursing care delivered by the nursing assistants working under Team Leader’s supervision.

Full time - Schedules:

• Evening shift 3:00 pm – 11:30 pm

Compensation: $27.50- 30.50 / hour base pay based on experience- plus any shift and weekend premiums.

LPN Duties:

• Administers and charts medications and treatments and adheres to all Pharmacy procedures.
• Makes rounds on each resident.
• Assesses for significant changes in residents’ condition.
• Supervises nursing assistants on the unit; evaluates work performance in terms of resident care, staff relations, efficiency of service, and intervenes when needed.
• Initiates and completes all necessary documentation and reporting.
• Communicates all order and status changes to the interdisciplinary team.
• Responsible for completing nursing notes according to Medicare/Medicaid guidelines and Bethany Village charting guidelines.

LPN Qualifications:Due to our federal funding, we are a drug free workplace (including marijuana). A pre-employment drug screen is required for all positions.

• License Practical Nurse from accredited school of nursing.
• Long term care experience preferred, but not required.
