Job Description
- Conduct clinical assessments and reassessments with referred customers.
- Review medical and other health-related documents and conditions using standard procedures.
- Review prior documentation as applicable.
- Conduct biopsychosocial assessments using approved questionnaires and techniques.
- Perform Social Security Administration sequential evaluations for disability determination.
- Conduct mental health evaluations for customers needing assessments.
- Identify customer functional limitations and determine functional capacity outcomes.
- Request or conduct additional assessments to support functional capacity determinations.
- Coordinate activities with Review Physicians and Evaluating Clinicians as needed.
- Collaborate with the Engagement Specialist for customer follow-up activities.
- Compile clinical assessment forms and prepare reports as required by HRA.
- Sign off on assessments and obtain customer signatures upon completion.
- Work with service staff to interpret clinical assessment findings for service planning.
- Use motivational interviewing techniques during assessments.
- Participate in team meetings to improve clinical assessment processes.
- Complete required training and adhere to policies and procedures.
Qualifications
- Must be a licensed or certified health professional (e.g., LCSW, CRC, LMHC) approved by HRA.
- At least four years of clinical assessment experience, including document review, biopsychosocial assessments, interviews, and SSA disability assessments.
- Experience in fast-paced, team environments with performance focus.
- Exceptional customer service, engagement skills, and experience in motivational interviewing and care management.
- Strong organizational skills and attention to detail.
- Experience with publicly-funded programs; TANF experience is a plus.
- Knowledge of relevant laws and regulations.
- Proficiency with electronic systems for duties.
