Job summary

Pinnacle Health & Rehab at Canton in Canton, Maine, is seeking a dynamic Charge Nurse (RN/LPN) to lead the second shift team. You will oversee certified nursing assistants, collaborate with direct care workers and ancillary departments, and develop patient-centered care plans in a skilled nursing/rehabilitation setting.

New graduates are welcome; Maine nursing license required. Part-time day and evening positions available.

Qualifications

Current Maine nursing license.
Experience in Long Term Care is a plus, but not required.
New graduates invited to apply.
Vaccinations must be up-to-date, and proof provided before hire.

Responsibilities

Provide leadership and oversight to the clinical team, ensuring high-quality patient care and adherence to facility policies and procedures
Collaborate with direct care workers and ancillary departments to ensure seamless coordination of patient care
Develop and implement individualized care plans for patients, ensuring patient-centered care and maximizing patient outcomes
Participate in quality improvement initiatives and committees to enhance patient care and facility operations
Maintain accurate and timely records, reports, and statistics
Supervise and mentor staff, providing guidance and support to ensure excellent patient care
