Pinnacle Health & Rehab at Canton in Maine is seeking a dynamic Charge Nurse (RN/LPN) to lead our second shift, overseeing a team of certified nursing assistants and coordinating care with direct care staff and ancillary departments.

As a Charge Nurse, you will develop individualized care plans, ensure patient-centered care, participate in quality improvement initiatives, and maintain accurate records while mentoring staff to uphold high standards of practice in a warm, home-like skilled nursing

Qualifications

• Current Maine nursing license.
• Experience in Long Term Care is a plus, but not required.
• New graduates invited to apply.
• Vaccinations must be up‑to‑date, and proof provided before hire.

Responsibilities

• Provide leadership and oversight to the clinical team, ensuring high‑quality patient care and adherence to facility policies and procedures.
• Collaborate with direct care workers and ancillary departments to ensure seamless coordination of patient care.
• Develop and implement individualized care plans for patients, ensuring patient‑centered care and maximizing patient outcomes.
• Participate in quality improvement initiatives and committees to enhance patient care and facility operations.
• Maintain accurate and timely records, reports, and statistics.
• Supervise and mentor staff, providing guidance and support to ensure excellent patient care.
