Job summary

Neogen Care is seeking a part-time Medical Social Worker to support home health patients on Oahu, HI. The role involves psychosocial assessments, care planning, and coordination with nurses and the care team in a home-based setting.

Ideal candidates hold CSWE-accredited degrees, an active LCSW license, and 1+ year healthcare experience, with CPR/BLS certification and a valid driver’s license. Prior home health experience is preferred.

Qualifications

• Bachelor’s and Master’s degrees in social work accredited by the CSWE.
• Must be licensed as an LCSW by the Board of Behavioral Sciences.
• Minimum of one year of health care experience.
• Home health care experience preferred.
• Excellent verbal and written communication, and strong organizational skills.
• CPR/BLS certification required.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Assess the psychosocial status of patients related to illness and environment and communicate findings to the RN.
• Conduct social evaluations and plan interventions.
• Maintain clinical records for patients referred to social work.
• Provide information and referrals for patients and families.
• Serve as liaison between patients, families, and community agencies.
• Collaborate with organization personnel to support patient care.
• Develop contracts with public and private agencies to assist patients and staff.
