Job summary

Qualifications

Responsibilities

• Conduct initial and ongoing assessments of patients' and residents' physical, emotional, and social needs.
• Develop and implement personalized care plans to address their needs and goals.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Provide counseling and support to patients and residents, as well as their families and caregivers.
• Facilitate communication among patients, families, and healthcare providers.
• Identify and address social determinants of health that impact patients' and residents' well-being.
