Job summary

AmeriStaff Nursing Services in Taylor, MI is seeking a licensed Speech-Language Pathologist to deliver in-home therapy, evaluating and treating patients with speech, language, voice, and swallowing disorders in their own homes. You will work part-time with a flexible schedule, developing individualized treatment plans, collaborating with a multidisciplinary team, and documenting progress and outcomes.

Qualifications

Must be a graduate of an accredited Speech-Language Pathology program.
Current Michigan SLT license required.
Minimum 1 year of experience in home health or clinical settings preferred.
Valid driver's license, car, and auto insurance.
Ability to lift up to 25 pounds occasionally; CPR certification preferred.

Responsibilities

Evaluate patients to identify speech, language, cognitive-communication, voice, and swallowing deficits.
Develop and implement individualized home-based treatment plans.
Provide skilled speech therapy interventions in patients' homes.
Monitor progress and revise plans to meet goals.
Educate patients and caregivers about treatment, safety, and home exercises.
Maintain accurate clinical documentation.
Complete assessments, progress notes, and discharge summaries.
Collaborate with physicians, nurses, therapists, and social workers.
Participate in case conferences and care planning.
Identify changes in patient condition and report findings.
Promote patient independence, safety, and quality of life.
