Qualifications

• Must have reliable transportation.
• Ability to read and follow written instructions in regard to the established plan of care.
• Ability to work both independently and with a team, sometimes having minimal supervision.
• Compassionate and patient-centered approach to care.
• Must possess a valid RN license in good standing with the state board of nursing.
• Minimum of 1 year of experience in home health or related healthcare field.
• Must be able to pass a background check and drug screen.
• Must have reliable transportation and valid insurance.
• Current CPR certification and TB.

Responsibilities

• Conduct comprehensive assessments of patients' chronic conditions.
• Provide services as ordered by the treating physician.
• Educate patients and their families on disease process management.
• Assist the Case Manager with care coordination (e.g., DME/HME).
• Document services and observations accurately and timely.
