Job summary

Signify Health, part of CVS Health, seeks a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. You will visit patients in their own homes, review medications and medical history, conduct physical assessments and assist in coordinating care with health plans.

Responsibilities include local travel to licensed states, optional virtual visits, and delivering diagnostic and preventive services as indicated.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations for patients in their own residences.
• Travel locally to licensed states as needed for visits.
• Conduct virtual visits when required.
• Perform diagnostic and preventive services (e.g., spirometry, etc.) as indicated.
• Engage with health plan members to review medications and medical history.
• Utilize iPad and clinical workflow; no prescription changes unless ordered by plan.
