Job summary

Disability Services of the Southwest is seeking an Attendant/Provider to deliver in-home care under supervision. The role focuses on care, warmth, and support aligned with each client's plan of care, on a part-time basis.

Responsibilities include assisting with daily living activities, ensuring client safety, maintaining notes, and attending required trainings. Must be 18+, have a HS diploma or GED, and reliable transportation to client sites.

Qualifications

• Must be at least 18 years old.
• High school diploma, GED, or pass a competency evaluation with three references.
• CPR and First Aid (if applicable per program).
• Cannot be a legal parent, foster parent, or spouse of a parent of a minor who recieves the services.
• Must have experience or training that demonstrates the ability to provide attendant care.
• Must not have any convictions for certain crimes described in stated criminal legislation relating to caring for the aged and disabled.
• Must have reliable transportation.

Responsibilities

• Provide direct care and supervision according to the client's individualized service plan.
• Ensure Client’s safety in their living environment.
• General household work only related to the care of the aged or infirm client.
• Assist with mobility, transfers, and ambulation when appropriate.
• Assist Client with Activities of Daily Living.
• Attend workshops, training and in-service sessions as required.
• Travel required for Client visits/hourly care and community activities as necessary.
• Maintain accurate service notes and documentation.
• Report accidents, injuries, incidents, or changes in condition to supervisors promptly.
• Adhere to agency policies, confidentiality requirements, and all applicable state regulations.
