Lutheran Social Services of Wisconsin and Upper Michigan is currently seeking an 18 hour per week HUD Service Coordinator to join our outstanding team in Marquette, MI.

The HUD Service Coordinator provides information and referrals to community connections to tenants who need supportive community services to maintain self-sufficiency. This position will cover properties in Marquette, MI; 12 hours a week at Cherry Creek Village and 6 hours a week at Oakwood Village.

Essential Duties and Responsibilities:

  • Provide general informal case management, including intake and referral services to all tenants needing such assistance when case management is not available.
  • Assessment of health, psychological, and social needs; development of an individually tailored case plan for services and periodic reassessment of the tenant’s situation.
  • Establish linkages with all agencies and service providers in the community to assure individualized, flexible, and creative services for the involved tenant(s).
  • Set up a directory of providers for use by both project staff and tenants.
  • Refer and link tenants to service providers in the general community for case management, personal assistance, homemaker services, meals on wheels, transportation, counseling, visiting nursing, preventative health screening/wellness, and legal advocacy.
  • Educate tenants on service availability, application procedures, and client rights, providing advocacy as appropriate.
  • Monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the individual.
  • Help tenants build informal support networks with other residents, family, and friends.
  • May provide training to project tenants in the obligations of tenancy or coordinate such training.
  • May educate other staff on issues relating to aging in place and service coordination.
  • Presents workshops or informative sessions to tenants on subjects of interest.
  • Demonstrate the ability to work with tenants, the agency, and the community positively as well as the ability to work independently.

Qualifications:

Education and/or Experience:

Bachelor’s degree in social work, sociology, psychology, or a related field or an equivalent combination of education and/or experience is required, with at least 3 years of related experience, including experience working with the older adult population. Working knowledge of supportive services and other resources for older adults and/or persons with disabilities is preferred.

Certificates, Licenses, Registrations:

The incumbent must have a valid driver’s license and reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record is required, and the ability to meet LSS auto insurance requirements.

Language Skills:

Ability to communicate effectively and efficiently in both verbal and written formats. Must document progress reports and clearly communicate to external customers the effectiveness of services.

Computer Skills/Technology:

Working knowledge of computers to allow access to computer systems and applications to complete timecards, read and respond to email, and complete required online training.

While performing the duties of this job, the employee is regularly required to bend, stoop, crouch, kneel, and crawl. The incumbent works in an air-conditioned office environment with a moderate noise level and may occasionally be exposed to chemical fumes and household allergens.

Travel: Day-to-day travel is required, up to 50% of the time on day trips.
