Qualifications

• Minimum of one year in a social work role, hospice or healthcare experience preferred.
• Licensed to practice Social Work in New Mexico.
• Current CPR certification, valid driver’s license, and reliable transportation with auto insurance.

Responsibilities

• Respond to referrals from the clinical team for social work support.
• Perform in-depth assessments to identify emotional, social, and economic concerns.
• Offer brief counseling and assist families with navigating available resources.
• Provide home visits and be available for on-call duties as required.
