Qualifications

Licensed to practice Social Work in New Mexico.
Current CPR certification.
Responsibilities
Respond to referrals from the clinical team for social work support.
Perform in-depth assessments to identify emotional, social, and economic concerns.
Offer brief counseling and assist families with navigating available resources.
Assist with planning, goal-setting, and connection to community-based services.
Keep accurate records and maintain documentation in accordance with care plans.
Participate in case discussions and contribute to interdisciplinary planning.
Provide home visits and be available for on-call duties as required.
Skills

Education
