Job summary

VNA Care is seeking a Home Health Registered Nurse Case Manager for a full-time role serving the Hingham, Hanover, Braintree, Weymouth area. The position requires an MA RN license, compassion for patients, and the ability to coordinate care across teams.

Expect travel within a defined territory as part of routine duties. Schedules are typically Monday–Friday, 8:00 am–4:30 pm, with benefits starting on day one including medical, dental, vision, and supplemental options.

Qualifications

• Current MA RN license in good standing.
• 1 year of med/surg experience; 1 year of acute care experience helpful.
• Willingness to travel within territory and complete patient visits as assigned.

Responsibilities

• Provide home health case management for assigned patients and families.
• Coordinate care plans with physicians, therapists, and support staff.
• Travel within Hingham, Hanover, Braintree, Weymouth area to visit patients and schedule visits.
