Job summary

Deaconess Home Health in Jasper, IN is seeking a skilled Physical Therapist to join our home health team on a part-time basis, three days per week, 8-hour day shifts.

The incumbent will assess patients, develop individualized treatment plans, and work with families and caregivers to promote recovery and functional movement in the home setting. Mileage reimbursement is provided and there are no after-hours on-call requirements.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Active PT license in Indiana.
• Current CPR/BLS certification.
• 1 year PT experience preferred, not required.
• Valid driver's license and ability to travel to patient homes.

Responsibilities

• Conduct evaluations of patients’ mobility, strength, and functional status.
• Implement therapeutic exercises and modalities to improve function.
• Educate patients and families on exercises and safety at home.
• Monitor progress and adjust treatment plans as needed.
• Maintain accurate patient records per regulations and policies.
