Job summary

BAYADA Home Health Care is seeking a part-time Physical Therapist (PT) to perform home health visits in the New Castle County area of Delaware. The role requires a DE PT license and at least one year of PT experience, with EMR proficiency and strong time-management skills.

The PT will develop and implement individualized care plans, conduct mobility and safety assessments, educate clients and family members, and document all care activities.

Qualifications

• DE PT license required.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans.
• Educate clients and families on rehabilitative care.
