Job summary

BAYADA Home Health Care is seeking a part-time Outpatient Home Health Physical Therapist to provide 1:1 PT services in Carneys Point Township, New Jersey. You will perform patient visits, develop care plans, and help clients maintain independence in daily activities.

Qualifications

• Current DE Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Develop and implement individualized care plans.
• Educate clients and families on rehabilitative care.
• Document observations and interventions in EMR.
• Collaborate with nursing for multidisciplinary care.
