Job summary

Impact Healthcare LLC is seeking a part-time Physical Therapist for home health services in Visalia/Fresno, CA. You will evaluate patients in their homes and implement PT plans of care with a patient-centered approach.

Qualifications

• Active Physical Therapy License in California.
• 2–3 years in Home Health and Hospice PT experience.
• Strong time management and organizational skills.
• Excellent verbal and written communication.
• Reliable transportation with valid license and insurance.
• Current CPR certification.
• Ability to perform physical aspects of the role.

Responsibilities

• Provide PT services per plan of care in patients' homes.
• Initiate therapy programs and train family/caregiver members.
• Identify needs for other home health services and refer as needed.
• Prepare and submit progress summaries and goals.
• Deliver home exercise programs to patients and families.
