Job summary

HCC, Healthcare at Home, is seeking a licensed Physical Therapist to support patients in their homes in the Visalia/Fresno area. The role requires compassion, strong communication, and a commitment to high-quality home-based PT care.

The PT will assess, plan, and implement interventions per a care plan, educate patients and families, and coordinate with the care team to optimize outcomes. This is a part-time position serving the Fresno/Central Valley region.

Qualifications

• Licensed Physical Therapist in California.
• 2–3 years of Home Health and Hospice PT experience.
• Strong time-management and organizational skills.
• Ability to follow verbal and written instructions.
• Driver's license and auto insurance; reliable transportation; CPR certification.

Responsibilities

• Provide physical therapy services to patients according to a written plan of care.
• Initiate PT program and train family/caregivers and staff as needed; participate in case conferences.
• Identify patient and family needs for other home health services and refer as appropriate.
• Prepare and submit clinical and progress summaries based on goals attainment.
• Deliver home exercise programs to patients and caregivers when indicated.
