Job summary

Pasco Southwest Home Health in Grand Junction, CO is seeking a Part-time Home Health Caregiver to provide essential personal care to clients in their homes. This on-site role supports independence and dignity, with shifts Monday through Friday and weekend availability as needed.

The role requires a high school diploma, completion of appropriate training, and at least six months of aide experience in home care. Valid Colorado driver’s license and reliable transportation are required.

Qualifications

• Six months or more of aide experience in a home care setting is preferred.
• Knowledge of personal care and safety practices in a home care setting.
• Ability to communicate effectively, verbally and in writing.
• Ability to establish and maintain effective working relationships with clients and families.

Responsibilities

• Provide personal care including baths, oral hygiene, and assistance with dressing.
• Assist with toileting and skin care to prevent breakdown.
• Maintain cleanliness and orderliness of the client’s living area.
• Plan and prepare nutritious meals and assist with feeding as needed.
• Observe and document care provided and any changes in condition.
