Job summary

HonorHealth is seeking a Float Pool Registered Nurse - Case Manager for a part-time, in-person role across various Phoenix locations. The RN will coordinate care for high-risk patients, discharge planning, and transitions of care, collaborating with physicians and the care team to ensure timely and appropriate services.

Required: RN license, 3 years hospital clinical experience. 5 years in RN Case Management and CCM preferred; outpatient experience and CCM certification preferred.

Qualifications

• Associates in Nursing is required at minimum.
• Bachelors in Nursing or healthcare-related field preferred.
• 3 years of clinical hospital experience required.
• 5 years as an RN in Case Management preferred.
• 7 years as a Registered Nurse (RN) preferred.
• Experience in outpatient settings with PCP, Home Health or Rehab preferred.
• RN licenses/compact state licensure required; CCM certification preferred.

Responsibilities

• Guides care coordination for high-risk acute care patients and discharge planning.
• Analyzes medical records to verify admission, treatment, and LOS per guidelines.
• Advocates for patients and families and liaises with the care team.
• Collaborates with physicians and coordinates resources for timely services.
• Coordinates transitions of care and maintains relevant logs and data.
• Performs duties per hospital policy and regulatory standards.
