Family Health Center of Worcester, Inc. is seeking a part-time Nurse Practitioner to support the Transition of Care and Medication Refill Team in Worcester, MA.

Qualifications

• Graduate of ANA accredited Nurse Practitioner program.
• Master’s degree in nursing required.
• Minimum of 2 years of experience
• Nationally certified as a Family Nurse Practitioner
• Massachusetts licensed
• Current DEA/Mass Controlled Substance license for prescriptive authority. BLS required.

Responsibilities

• Refill medications for patients without PCP to reduce hospital admissions.
• Telehealth precepting and scheduling of in-person/telehealth visits.
• Provide comprehensive family health care including assessment, diagnosis, and treatment.
