Sanford Health’s Woodsedge facility in Bemidji, MN is seeking a Licensed Practical Nurse (LPN) to provide professional nursing care for residents in assisted living settings under supervision. You will demonstrate nursing competencies, communicate effectively, and support the care team in achieving patient-centered outcomes.

Qualifications

• Must have graduated from an accredited school or college of nursing leading to licensure as a Licensed Practical Nurse.
• Long term care, sub-acute care, medical/surgical experience preferred.
• Requires current licensure as a practical nurse in the state/states of practice.

Responsibilities

• Provide professional nursing care for residents under supervision of a RN/physician.
• Administer medications and treatments as ordered, per care plan.
• Document resident interactions, care outcomes and communications.
• Contribute to assessments and care planning within interdisciplinary team.
