Qualifications

• Must possess a valid driver's license.
• Must have a 4 door sedan, minivan, or SUV under 15 years old.
• Must be current on personal auto insurance.
• Must have an active cell phone plan; if prepaid and not on autopay, provide proof of 6 consecutive timely payments.
• Must be a U.S. citizen or resident with a TIN (SSN, EIN, or ITIN acceptable).

Responsibilities

• Transport injured workers to appointments in your own vehicle when scheduled.
