Job summary

Qualifications

Responsibilities

• Provide direct care and support to individuals in their homes and in the community.
• Assist with activities of daily living including personal hygiene, meals, light housework, and basic skill-building.
• Encourage social engagement and community participation.
• Follow individualized support plans and document progress toward goals.
• Maintain accurate daily activity logs and incident reports, including health events.
