Job summary

Young Adult Institute in Brooklyn is seeking staff to provide person-centered support and supervision for individuals in the Linden Boulevard IRA program. The role includes assisting with daily living activities, social engagement, and crisis management in a Part-Time schedule on Sundays and Mondays from 3:00pm to 11:00pm.

Responsibilities

• Assists individuals in maximizing their strengths and building positive relationships.
• Supports individuals to meet personal goals and encourages socializing through person-first language.
• Responsible for supervision, protection, and care of individuals following program guidelines.
• Provides health, wellness and safety supervision and manages crisis situations per policies.
• Provides support per individualized service and behavior support plans per OPWDD Core Competencies.
• Implements person-centered plans including IPoPs, BSPs and Life Plans for all individuals.
• Maintains a clean, safe and organized therapeutic environment.
• Uses safe handling procedures and infection control protocols.
• Assists with tasks including ADLs, medication administration, and meal preparation.
• Assists in communication, self-expression and dispute resolution among individuals.
• Facilitates ethical decision making and supports balanced meals within dietary guidelines.
• Transports and escorts individuals to activities, appointments and community sites.
• Assists with purchases and safeguarding personal funds as needed.
• Develops clinical knowledge for person-specific needs and behavior support.
• Implements Behavior Support Plans as written by BIS.
• Collects and summarizes data on observable behavior.
• Reports changes in behavior to clinical/leadership teams.
• Works as part of a team and maintains a solution-oriented approach during conflicts.
• Completes trainings and certifications to ensure compliance with YAI policies.
• Reviews communication logs and documentation for daily operations and billing.
