Job summary

Community Health Center, Inc. is seeking a Part Time Dietitian to manage nutrition care for patients at Stamford and Norwalk sites. The role includes delivering Medical Nutrition Therapy, leading nutrition groups, and educating staff and community partners.

The position requires a CADE-accredited dietetic internship (900 hrs) and an RD/RDN credential, ideally with a Master’s in a related field. Telehealth options are available for Stamford, with on-site duties in Norwalk.

Qualifications

• Baccalaureate degree in Food and Nutrition, Dietetics, Nutrition Sciences or comparable majors required.
• Successful completion of a CADE-accredited dietetic internship of no less than 900 hours under supervision of a Registered Dietitian.

Responsibilities

• Provide effective nutrition counseling utilizing the Nutrition Care Process.
• Lead nutrition groups and provide nutrition education to staff.
• Engage in community outreach and nutrition education to external agencies.
• Document nutritional assessment and plan of care in the electronic health record.
• Promote Patient Centered Medical Home through nutrition care.
