Ryders Health Management seeks a compassionate part-time Dietitian for Bel-Air Manor in Newington, CT to provide comprehensive nutritional care to residents in a skilled nursing setting. You will assess nutritional needs, develop care plans, deliver medical nutrition therapy, and collaborate with an interdisciplinary team to promote resident well-being and regulatory compliance.

Qualifications

• Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR).
• Current Connecticut Dietitian-Nutritionist certification as required by state regulations.
• Bachelor's degree in Dietetics, Nutrition, or a related field from an accredited college or university.
• Previous clinical nutrition experience in long-term care or skilled nursing preferred.
• Knowledge of medical nutrition therapy, therapeutic diets, nutrition assessment, and care planning.
• Experience working with older adults and interdisciplinary healthcare teams.

Responsibilities

• Conduct comprehensive nutritional assessments for residents upon admission and as required thereafter.
• Develop, implement, and monitor individualized nutrition care plans based on medical conditions and goals.
• Provide medical nutrition therapy and recommendations for therapeutic diets and supplements per physician orders.
• Monitor residents' nutritional status, weight, intake, hydration and labs.
• Collaborate with nursing, physicians, speech therapy, rehab and food services.
• Provide nutrition counseling and education to residents and families.
• Review and adjust care plans for weight changes or intake issues.
• Ensure documentation in medical records is accurate and timely.
• Stay current with regulatory requirements and best practices in nutrition in long-term care.
