Job summary

The Center for Rehabilitation & Nursing at Washington Township is seeking a part-time Dietitian to join our committed team. The role offers 24 hours per week with a pay rate of $30.00-$35.00 per hour and a friendly work environment.

Responsibilities include assessing residents' nutritional needs, creating and evaluating care plans, and providing education to residents, families and staff while collaborating with interdisciplinary teams. State licensure and RD registration are required.

Qualifications

• Registered by the Commission on Dietetic Registration (CDR).
• Certification / License in good standing within the state of practice.
• Knowledge of state survey processes and compliance with nutrition care regulations.
• Strong organization and communication skills.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop a plan of care within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake and comorbidities.
• Provide nutrition education and counseling to residents, families and staff.
• Collaborate with interdisciplinary teams to develop a nutritional health plan for the patient's overall wellbeing.
• Adhere to facility policies and procedures and to CMS, State and Federal regulations.
• Assist and participate in interdisciplinary care conferences.
• Assist the dietitian in duties as assigned.
