Job summary

The Center for Rehabilitation & Nursing at Washington Township is seeking a Dietitian to join our dedicated team on the day shift. You will assess residents/patients, develop and document nutrition plans, and collaborate with our care team to deliver individualized nutrition care.

You will prepare meal plans tailored to medical, nutritional, age, cultural, religious, and ethical needs; provide weekly updates, ensure timely documentation, and participate in rounds to support optimal resident

Qualifications

• Must be a Registered Dietitian.
• Bachelor's Degree in Dietetics from an accredited college or university.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop and document the plan within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities.
• Prepares individualized meal plans that consider the needs of the patient including; medical, nutritional, age, cultural, religious and ethical concerns.
• Provides updates on the nutritional status of each patient weekly as part of team rounds/meetings and as requested.
• Collaborates with medical professionals to assure patient nutrition needs are met.
• Completes all required documentation and reports in a timely manner, maintaining compliance with the policies and procedures

Dietitian Responsibilities

QUALIFICATIONS For a Registered Dietitian

• Must possess, as a minimum, a Bachelor’s Degree in Dietetics from an accredited college or university.
• Must be a Registered Dietitian. Must have training in food management, diet therapy, etc.
