Job summary

The Dietitian will collaborate with physicians and interdisciplinary teams to ensure comprehensive nutrition care and adherence to clinical standards within a home health

Qualifications

• Registered dietitian with state licensure required.
• Bachelor's or higher in Food & Nutrition required; RD preferred.
• Clinical Dietetics experience is advantageous.

Responsibilities

Assess patients' nutritional needs, develop and implement personalized dietary plans, educate patients on healthy eating habits, instruct on post-discharge diets, monitor nutritional status, evaluate referral orders, document in medical records, conduct calorie counts, review and revise menus, and assist with special dietary needs.

Qualifications

Registered by the Commission on Dietetic Registration, state licensure required, Bachelor's or higher degree in Food & Nutrition, and preferred membership in the Academy of Nutrition and Dietetics. Clinical Dietetics experience is advantageous.

Skills

Strong communication with patients, physicians, and interdisciplinary teams; ability to evaluate and adapt nutritional plans; attention to detail in documentation; and knowledge of nutritional standards.
