Job summary

Jobtailor in Houston, TX is seeking a registered dietitian to conduct comprehensive nutritional assessments, develop, implement, and monitor individualized nutrition care plans for residents, and provide nutrition education and counseling.

You will collaborate with physicians, nurses, therapists, and the interdisciplinary care team while ensuring compliance with CMS and state regulations, maintaining accurate documentation, and monitoring weight trends to support resident outcomes.

Qualifications

• Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program.
• Registered by the Commission on Dietetic Registration (RD).
• Current state licensure/certification as mandated.
• Minimum 1 year clinical experience preferred.
• Excellent communication with patients, families, and healthcare teams.
• Ability to apply critical thinking to develop and monitor improvement plans.
• Proficiency in general nutrition interventions.
• Familiarity with state survey processes and nutrition-regulation adherence
• Competency in electronic charting systems.

Responsibilities

• Conduct comprehensive nutritional assessments.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition education and counseling.
• Collaborate effectively with physicians, nurses, therapists, and other members of the interdisciplinary care team.
• Ensure compliance with all applicable state and federal regulations, including those related to the Centers for Medicare & Medicaid Services (CMS) and the Department of Health (DOH).
• Maintain appropriate documentation.
• Monitor all residents for changes in weight weekly and/or monthly to identify potential changes in nutrition or hydration status.
• Participate in quality improvement initiatives to enhance nutrition care services and resident outcomes.
• Perform other tasks as assigned and within scope of practice.

Requirements

• Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program.
• Registered by the Commission on Dietetic Registration.
