Job summary

Saint Francis Hospital and Medical Center, part of Trinity Health Of New England, seeks a Crisis Clinician to deliver psycho-social services in ED and Behavioral Health. The role includes comprehensive assessments, patient advocacy, and coordination with a multidisciplinary team to support safe discharges.

The successful candidate will have an MSW, CT licensure, and strong communication skills. Per diem scheduling allows weekend coverage and flexible hours within a rotating shift.

Qualifications

• MSW degree from an accredited School of Social Work or Behavioral Health Field required.
• CT licensure in appropriate professional designation required (Clinical Social Worker, Psychologist, Marriage and Family Therapist, or Professional Counselor).
• Excellent written and verbal communication, knowledge of community resources, social work theory and practice.

Responsibilities

• Performs psycho-social assessments for adult, adolescent, and child patients in ED and Inpatient Behavioral Health.
• Provides consultation to other health care providers to manage psychosocial problems.
• Acts as patient advocate to ensure appropriate care and safe discharge planning.
• Communicates effectively with patients, families and the clinical team to coordinate care.
