Job summary

Phoenix Home Care and Hospice in Springfield, MO seeks a Certified Occupational Therapy Assistant (COTA) to work under a Licensed OT in home health settings. You will provide therapy services, document care, and collaborate with the care team to update plans of care and goals as patients improve, while ensuring regulatory compliance and agency standards.

Part-time and PRN schedules are available, with orientation and training tailored to new hires.

Qualifications

• COTA certification is required.
• Experience in home care or rehab is preferred.

Responsibilities

• Provide assigned occupational therapy services under the supervision of a Licensed Occupational Therapist.
• Perform direct occupational therapy services as ordered by a physician and per the Plan of Care.
• Maintain compliant documentation of all care and services.
• Collaborate with team to review and update patient plans of care and goals.
• Ensure adherence to agency standards and regulations.
