Job summary

Community Ventures In Living Ltd in Valparaiso, IN is seeking a Community Support Coordinator to provide direct and indirect support to individuals with developmental disabilities. The role involves coordinating services, coaching in life skills, and transportation to appointments within a flexible 20-hour workweek.

You will document progress, communicate with families and team members, and help access community resources while maintaining respectful, person-centered support.

Qualifications

• Experience working with people with disabilities.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Valid driver’s license, proof of auto insurance, and vehicle registration required.
• Reliable transportation.
• Ability to communicate clearly in a variety of situations is essential.
• 18 years of age or older.

Responsibilities

• Promote autonomy and self-determination.
• Actively support or promote socially valued roles within the individual's community.
• Assist with daily living tasks and life skills development.
• Provide indirect care services to those who work directly with individual served.
• Promote a positive public perception of both the individuals served and of oneself.
• Make scheduled and unscheduled visits to the homes of individuals receiving services.
• Participate in staff meetings and training.
• Implement, directly or through delegation, the goals and objectives of the service plan.
• Assist in accessing community resources.
• Monitor and address medical needs of individuals served.
• Communicate effectively with individuals, families, and other team members.
• Provide transportation to appointments and activities.
• Maintain accurate and current individual records and documentation.
• Attend team meetings and provide updates on individual progress.
